If you go to www.coastalobesity.com, you will find a form there that
is EXTREMELY comprehensive. You might want to try printing that out
and using it as a basis for the info to submit to the insurance
company. But with Medicaid or Medicare, I don't believe they even
give preapproval - you just find a surgeon/hospital who accepts it
and then they will bill Medicaid/Medicare directly. I'd check with
your surgeon's office to be sure.
